What we collect
- Contact info you give us (name, email, phone) when you book a session, fill out the contact form, or sign up for our mailing list.
- Health intake info at your first visit (contraindication screening, basic medical history relevant to Neubie treatment). This is collected on paper or via secure intake form and stored in our locked clinic records.
- Session records (SOAP notes) documenting your treatment. These are retained per state record-retention requirements.
- Payment info processed by our payment provider (Stripe). We do not store credit-card numbers ourselves.
- Anonymous web analytics (page visits, time on site) — no personal identifiers, no cross-site tracking. We do not use Google Analytics.
What we don't collect
- Social Security numbers
- Credit-card numbers (handled entirely by Stripe)
- Tracking cookies for advertising
- Information from third-party data brokers
How we use it
- To schedule and deliver your sessions safely
- To send appointment confirmations and reminders
- To document treatment per clinical standards
- To process payment for services rendered
- To respond to your inquiries
- To share records with your attorney in LOP cases (with your written authorization)
Who sees your information
- Lexis Peptora Recovery Lab staff who need it to do their job
- Our supervising chiropractor of record (for clinical sign-off)
- Our payment processor (Stripe), under industry-standard PCI-DSS controls
- Your attorney (LOP cases only, with your written authorization)
- Government agencies if compelled by law (subpoena, court order)
We do not sell your information. We do not share it with marketing partners. We do not use it to train AI models.
HIPAA status
Lexis Peptora operates as a cash-pay and Letter-of-Protection clinic. We do not transmit standard HIPAA-defined insurance transactions electronically. Under the strict federal definition of "Covered Entity," we are generally not a HIPAA-covered entity. However, we voluntarily apply HIPAA-compatible privacy practices to clinical records as a best-practice posture, and Georgia state medical-records privacy law (O.C.G.A. § 31-33) applies to your records regardless of federal HIPAA status.
Data retention
Clinical records are retained for the period required by Georgia law for chiropractic and allied-health clinical records (currently 10 years for adult records). Non-clinical contact info is retained as long as your account is active plus 1 year for general business purposes, then deleted upon written request.
Your rights
You have the right to:
- Request a copy of your records
- Request correction of inaccurate records
- Request deletion of non-clinical contact information
- Withdraw consent for future communications
